MEMBERSHIP FORM TEMPLATE - For Club use
Following changes to the Privacy Act, the Archery Australia Board developed a Membership Form Template.
The template contains the basic mandatory information that must be collected from people joining or renewing their membership with your club and a mandatory Declaration by the Applicants that complies with the new Privacy Act.
Clubs should utilise this template and ensure that they include the Declaration by Applicant, with the wording to that declaration left unchanged.
A Club can add any other additional information it wishes to collect or detail any other requirements or conditions it wishes to impose upon its members.
It must be stressed clubs should ensure anyone not joining or renewing online should complete a Club Membership Form at the time of joining or renewing.
The club should ensure that it keeps the original of the completed and signed copy of the Membership Form on file.
The template is being provided in Word format so a Club can add its own logo and other information to customise it to the Club's needs.
